What do I need to bring to the signing appointment?

Please have one of the following documentation with you at the time of the signing that must be current or if expired, issued within the past five years, and bear a serial or other identifying number. Acceptable identification documents include:

1. Florida driver's license or identification card issued by DMV

2. U.S. passport issued by the U.S. Department of State

3. Foreign passport if stamped by U.S. Citizenship and Immigrations Services (USCIS)

4. Driver's license or non-driver's ID issued by another state or territory

5. Driver's license officially issued in Mexico or Canada

6. U.S. military ID

7. Inmate ID issued on or after January 1, 1991, by the Florida Department of Corrections or Federal Bureau of Prisons (but only to identify prisoners in custody)

8. A sworn, written statement from a sworn law enforcement officer explaining that an inmate's IDs were confiscated upon incarceration, and that the person named in the document is the person whose signature is to be notarized

9. Permanent resident card, or "green card," issued by the U.S. Citizenship and Immigration Services (USCIS)

10. A veteran health identification card issued by the U.S. Dept. of Veterans Affairs
